You will receive this message if you have incorrectly entered your password into the assessment survey home page.


  1. If you have not logged in to the assessment survey site previously, look for your temporary password in your email invitation. Check to make sure you are entering the temporary password with no spaces before or after the first and last character in the password prompt. (It must be entered exactly as it is found in your email invitation.)
  2. If you have logged in to the assessment survey site previously, you were required to change your password at that time. If you have forgotten your changed password, go to, and click the “Forgot Your Password” link. You will be prompted to enter your email address. Click on “Get Password” Your password will be reset and your new password will be emailed to you.
